Una vez finalizado el estudio básico de fertilidad, indicamos que tratamiento ó técnica será la adecuada. Los tratamientos más habituales son:
- Inducción de la ovulación.
- Programacion de coitos en dias de máxima fertilidad.
- IAC (Inseminación Artificial Conyugal): concentración espermática de 5-6 millones y trompas permeables.
- FIV (Fecundación In Vitro): concentración espermática de 2-4 millones.
- ICSI (Inyección Espermática Intracitoplasmática): concentración espermática menor de 2 millónes
- Otras técnicas que conlleven la utilizacion de banco de semen, donacion ovocitaria ó embrionaria... cultivo embrionario a blastocisto, hatching asistido, diagnóstico genético preimplantacional.......etc.
Once reviewed the results of the diagnostic study, we propose the most appropiated Assisted Reproduction Technique for each couple:
- Artificial Insemination by husband (AIH): When the sperm concentration is 5-6 millions and fallopian tubes are permeable.
- In vitro Fecundation (IVF): When the sperm concentration is 2-4 millions.
- Intracytoplasmatic Sperm Injection (ICSI): When the sperm concentration is less than 2 millions.
Other specific techniques which involve the use of donor semen, donor ovules or criopreserved embrios..... long embrios culture to blastocyst, assisted hatching, genetic preimplantation diagnosis,..etc.
To carry out any of the three above procedures women must previously undergo a controlled follicular stimulation by injecting her with FSH recombinant hormones, to induce an ovulation, of 1-2 follicles in the case of insemination, or more than 5-6 follicles for IVF or ICSI.
