Nuestro objetivo es triple: Conseguir una gestación, que el embarazo transcurra con normalidad y que nazca un bebé sano.Nuestro Equipo desea que la gestación se logre con el tratamiento ó la técnica más adecuada y en el periodo de tiempo más breve.
El estudio básico precisa unas pruebas sencillas:
- Elaboración de la Historia Clínica.
- Analítica en sangre y orina,
Determinación Hormonal Basal. (la analítica se debe de realizar entre el 2º y 5º día del ciclo mesntrual). hormonas tiroideas. Serología : hepatitis B y C, HIV, toxoplasma, rubeola y sífilis.Con ellas, se garantiza que el bebé no contraerá estas enfermedades capaces de traspasar la barrera placentaria, transmisión vertical, impidiendo el nacimiento de un bebé con estas enfermedades. Este estudio es muy fácil ya que toda la información se consigue con un simple análisis de sangre.
- Control Ginecológico: Exploración con toma citológica, ecografia transvaginal y exploración de mamas.
Una vez demostrado que el desarrollo de un embarazo puede acontecer sin el perjuicio para la madre, al no tener esta, enfermedades sistémicas, ginecológicas ni de transmisión sexual, ni perjudiciales para el bebé, (bienestar materno, bienestar fetal), se realiza el estudio básico de fertilidad, con el objetivo de poder diagnosticar cual de las técnicas de reproducción asistida es la más adecuada en cada caso. El estudio básico de fertilidad se basa en dos pruebas y excepcionalmente en otras de caracter especial:
Histerosalpingografía:
Realizada por un radiólogo. Se introduce un contraste a través del cuello uterino,con el objetivo de estudiar la cavidad uterina y las trompas, ya que son dos órganos fundamentales en la reproducción humana. La cavidad uterina es donde se va a desarrollar, acoger al bebé a lo largo de la gestación y será el motor del parto. Las trompas son las encargadas de captar el óvulo, que posteriormente será fertilizado por un espermatozoide, en su tercio distal. Para realizarse esta prueba debe de llamar al radiólogo al aparecer la menstruación.
Seminograma:
Esta prueba se realiza en el laboratorio de nuestro Centro, previa abstinencia sexual de 3-4 días. Además de estudiar los parámetros propios del seminograma; volumen, motilidad, morfología, nº espermatozoides vivos/muertos, etc, realizamos una técnica denominada Capacitación Espermática en la cual mediante unos procesos físico-químicos y un medio de cultivo,se hace la recogida de los espermatozoides más aptos para la fecundación, retirando aquellos malformados, con mala movilidad y muertos: espermatozoides óptimos para la fecundación. La muestra habitualmente la recoge en paciente en nuestro Centro para evitar su deterioro en el traslado. Seguimos los criterios de la ESHRE para su análisis.
The Basic study of fertility helps us to achieve three goals: to obtain a pregnancy, to confirm maternal well-being through-out the gestational period, and last, to deliver for a healthy baby.
After the first informative interview, couples understand the why of certain diagnostic procedures and the importance of involving both members of the couple.
Our team does not only wish the woman to become pregnant in the shortest period of time and with the most suitable treatment or technique; we also want to assure a maternal and fetal well-being, preventing woman from undesirable situations throughout the gestation period and delivering healthy newborn.
We firstly have to confirm that woman is healthy enough to achieve a pregnancy. For this purpose the woman must undergo some simple tests to demonstrate she is not affected by gynaecological disorders, making it impossible for her to achieve a gestation.
These tests are as follows:
- Clinical history.
- Serology: hepatitis B, C, toxoplasma, rubella, syphilis and HIV. These infective diseases are able to cross the placental barrier and infect the intrauterine baby.
- Blood and urine analyses
- Basal hormonal determination: blood must be obtained between the day 2 and 5 of the menstruation cycle.
This study is very easy since we can have all the information needed by a simple blood analysis
5.- Gynaecological exploration: briefly a pap smear, endovaginal us and breast exploration.
After being sure that pregnancy can be achieved without detriment neither for the mother nor the baby (maternal well-being/fetal well-being), we carry out the basic study of fertility to decide which of the Assisted Reproduction Technique is the most advisable for each case.
This basic study on fertility is based on two procedures:
Hysterosalpingography
To undergo this procedure, which is the radiography of the uterus and the fallopian tuba, the woman must visit the radiologist when her menstrual flow starts. The radiologist introduces a contrast material through the cervix to examine the uterine cavity and the fallopian tuba, since they are two basic organs in the human reproduction. The fallopian tubes will capture the ovules, which later are fertilized by the sperm in their distal third. The uterus will house the baby throughout the pregnancy and afterwards will be the motor of the delivery.
Spermiogram:
This procedure is undertaken in our center. The male must come to the institute and directly collect his semen here, after 3-4 days of sexual abstinence. Besides studying the volume, motility, morphology, number of the alive/dead spermatozoa, etc.,(which are the parameters tested in a seminography), we also carry out an Spermatic Capacitation. This technique, by means of physical-chemical processes and a culture medium, selects the more viable spermatozoa, removing those presenting malformations, poor motility and those taht are dead: Optimal spermatozoa for fecundation (REM). We follow the criteria established by the European Society of Human Reproduction and Embryology for REM analysis.
